The book covers Onision's life starting from the day he was born. The book abruptly ends on Chapter 22 with Onision announcing he is tired of writing it. The story ends with him being a 19-year-old in Air Force basic training. The last chapter consists of Onision listing lessons he learned in recent years of his life.
It was speculated Onision named the book "In Real Life" because the name of the 2021 Discovery+ Documentary made about the accusations against himself and his husband was named "Onision: In Real Life". It was thought he hoped people searching for information on the show would find his book instead. Onision explained he named it this because it is about his real life. (Source)
In December 2022, Onision announced he is publishing In Real Life to purchase digitally through iTunes. This new version of the book included a bonus chapter and art. The release date was set for March 23, 2023. (Source)
On February 24, 2025, Onision quietly release a hardback physical copy of the book through Amazon. This version featured a new cover. Both releases of the book featured AI generated images on the covers.
